PURPOSE OF POSITION: A Social Case Worker I at Catholic Charities: Provides professional case management services to program clients to improve client stability, self-sufficiency and personal growth and development. Travels throughout South Weld County to provided in home case management to communities aging older adults. Maintains office hours for South Weld, Fort Lupton satellite location. Applies appropriate techniques and standards in program development and service delivery of case management services to clients under close supervision. Implements program objectives and performance standards as directed by supervisor/ management Performs comprehensive needs assessment of clients within established guidelines through interviewing, record review, and other appropriate methods. Maintains complete, accurate and up-to-date case records and files. Maintains collaborative working relationships with government and community-based organizations for advocacy, education and resource development. Advocates and interacts with other service providers on behalf of participants. Maintains accurate and timely program statistical data to meet reporting requirements. Enters data and generates reports. Demonstrates skills and sensitivity to the cultural/linguistic differences of staff, clients and community. Requirements Bilingual in English and Spanish strongly preferred. Can stay calm and kind when others are upset Thrive in a fast-paced, sometimes chaotic environment Have a good sense of humor, and strong resilience in the face of challenges EDUCATION and/or EXPERIENCE Bachelor's degree (BA/BS) in social work or related field from an accredited college or university; and at least 1-year related experience and/or training working with seniors, veterans and/or disabled or an equivalent combination of education and experience. COMPENSATION & BENEFITS: Pay: $23 per hour. Eligible for $1 bilingual differential. Training: We provide a robust training curriculum that will support our employees throughout their career. Training provided within the first year of employment includes: De-escalation & Safety; Mental Health First Aid; Substance Abuse & Mental Health; Human Trafficking; Trauma Informed Care; and many more.
