candidates.Job Summary:The Clinical Case Manager exists to provide
alcohol, drug, and mental health treatment by evaluating the mental
health needs of those in the Adult Out Patient Clinics, providing
group counseling and individual therapy as needed. This position
provides or arranges case management services in furtherance of
treatment goals, maintains thorough patient records and coordinates
with the psychiatrist to ensure good care and proper medication
procedures are being followed for all dual diagnosed clients. This
position works in collaboration with a multi team including the
Associate Director, the Residential Coordinator, Primary Substance
Use Disorder Counselors, and Behavioral Health Technician staff.
This position evaluates the mental health needs of men and
determines when to consult further with the psychiatrist regarding
the disposition of such patients. This position educates and
mentors other staff in regard to a client's day-to-day mental
health needs.Key Responsibilities:
- Identifies dually diagnosed patients and coordinates effective
treatment interventions.
- Determines when a patient needs to be referred for psychiatric
evaluation and coordinates appointment schedule for the psychiatric
team.
- Provide counseling such as group or individual interventions as
necessary.
- Contributes to the development of person-centered care
plans.
- Maintains current and accurate records as required by
policy.
- Collaborates with external stakeholders such as the Department
of Human Services and Criminal Justice systems.
- May conduct abstinence monitoring. Work Location:Onsite - this

- A master's degree in human services, psychology, counseling,
social work, or a directly related field from an accredited
institution.
- Two (2) years of professional experience working with dual
diagnosed patients.
- Possess a valid registration in good standing with the Colorado
Department of Regulatory Agencies (DORA) as Licensed Professional
Counselor (LPC), Licensed Social Worker (LSW), Licensed Clinical
Social Worker (LCSW), or Licensed Marriage and Family Therapist
(LMFT). Applicants must meet minimum qualifications at the time of
hire.Preferred Qualifications:
- Three (3) years of professional experience working with dual
diagnosed patients.
- Experience with and knowledge of running evidence-based
counseling groups with dually diagnosed patients. Knowledge, Skills
and Abilities:
- Well-developed knowledge and skills in care coordination for
the dually diagnosed population.
- Ability to apply critical thinking and conflict resolution
skills using creative approaches.
- Strong organizational and time management skills to meet
deadlines.
- Ability to work with diverse patient populations and be
nonjudgmental and respectful of other's beliefs, values, and
culture.
- Ability to comply with established rules, policies, and
procedures.
- Ability to effectively and efficiently manage a diverse
workload in a fast-paced, rapidly changing regulatory
environment.
- Proficient in Microsoft Office programs and ability to learn
software programs.
- Ability to communicate effectively, both in writing and
orally.
- Ability to establish and maintain effective working
relationships with employees at all levels throughout the
institution.
- Outstanding customer service skills.
- Ability to establish boundaries with clients and exercise
